Course Application -
Students in a NYS EMS Agency, please put your agency name, agency code, your name, and email address in the boxes and have your authorized agency official sign and date.
Students NOT in a NYS EMS Agency, please put your name, email address and phone number in the boxes.
NYS Verification Forms -
Students in a NYS EMS Agency, please make sure you print out the NYS Verification Form and put your name, agency code, agency name, sign the bottom of the form and have your authorized agency official put their name, EMT number if that person has one and sign the bottom of the form too.
Students NOT in a NYS EMS Agency, please put your name and sign the bottom of the form.
The course application and NYS Verification MUST be mailed, or dropped off, to the Suffolk County EMS Division together with a check or money order.
DO NOT PUT THE SUFFOLK COUNTY COURSE NUMBER DOWN AS THE COURSE NUMBER. The Suffolk County EMS Division will put the NYS number down. Thank you.
