Students in a NYS EMS Agency, please put your agency name, agency code, your name, EMT number and email address in the boxes and have your authorized agency official print, sign and date.
Students NOT in a NYS EMS Agency, please put your name, EMT number, email address and phone number in the boxes.
NYS Verification Forms -
Students in a NYS EMS Agency, please make sure you print out the NYS Verification Form and put your name, EMT number, agency code, agency name, sign the bottom of the form and have your authorized agency official put their name, EMT number if that person has one and sign the bottom of the form too.
Students NOT in a NYS EMS Agency, please put your name, EMT number and sign the bottom of the form.
DO NOT PUT THE SUFFOLK COUNTY COURSE NUMBER DOWN AS THE COURSE NUMBER. The Suffolk County EMS Division will put the NYS number down. Thank you.
