Situational Awareness – Important Information Regarding Emergency Transfer of Patients by Ambulance from Skilled Nursing Facilities to Hospitals
Within our health care system, a new form has been created by the Suffolk Care Collaborative to document and transfer critical medical informational elements for patients transported by ambulance from Skilled Nursing Facilities / Long Term Care Nursing Homes to hospital Emergency Departments (ED). EMS providers are not responsible for completing the form, and this form does not replace the need for you to document on the NY State PCR or ePCR equivalent. Rather, we want you to be aware of the form so that you can appreciate its importance, and to ensure that the form is made available to you by the health care facility staff prior to departing the facility. In summary, this form was created to address hospital ED physician and staff concerns regarding difficulties in trying to quickly access critical medical information in an often voluminous medical chart. This form does not replace the chart, but becomes party of the chart to encapsulate critical medical elements in one easy-to-reference document.
