COURSE SCHEDULE
This is a New York State, Bureau of EMS and Trauma Systems Specialized Courses Course. Attendance at all session is mandatory. The course will cover PALS, AMLS, and ACLS Original Courses, Skills, AHA CPR, Trauma and Psychiatric, and Geriatrics and Immunology to cover Core requirements.
To register, please print out the course application with the NYS Verification form. CME Coordinators are required to sign the course application and NYS Verification form. This course is only for Suffolk County EMS Division CME Participants. The cost is $525.00 for the books and certifications.
FEMA 100, 200, 700 and HazMat Awareness are required to be submitted. In July 2024 NYS changed the HazMat Policy. FEMA 5A no longer counts. Below you will find three (3) options. Please pick one. Firefighter 1 counts as long as it is from 2003 to the present.

Students in a NYS EMS Agency, please put your agency name, agency code, your name, EMT number and email address in the boxes and have your CME Coordinator print, sign and date.
NYS Verification Forms -
Please make sure you print out the NYS Verification Form and put your name, EMT number, agency code, agency name, sign the bottom of the form and have your CME Coordinator put their name, EMT number if that person has one and sign the bottom of the form too
DO NOT PUT THE SUFFOLK COUNTY COURSE NUMBER DOWN AS THE COURSE NUMBER. The Suffolk County EMS Division will put the NYS number down. Thank you.
